Information about your rights to get your medical record

Remember that consumers have the right to --

(1) Ask to see and get a copy of their health records from most doctors, hospitals, and other health care providers such as pharmacies and nursing homes as well as from your health plan

(2) Get the copy of their record in an electronic copy or paper copy format depending if the plan or provider is able to do so.
